29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) 200 Old Tappan Road, Old Tappan NJ 07675 (6 on12 pitched roof) An employee engaged in sheathing activities using a portable extension ladder to access a 6 on 12 pitched roof was exposed to fall hazards of approximately 20ft to the asphalt below as the ladder side rails did not extend 3ft above the upper landing surface. Condition observed on or about 10/21/2022.
