29 CFR 1904.39(a)(2): The employer failed to report within twenty-four (24) hours of occurrence of the in-patient hospitalization and amputation of two fingers of one or more employees, as a result of a work-related incident. On or about October 20, 2022 at or near the location of South Third Street and Bridgeport Drive an employee had a jumping jack compactor fall off a trailer landing on his hand resulting in the amputation of his right middle fingertip and right ring fingertip. This resulted in the overnight admittance to the Hospital. Neither the hospitalization nor the amputation was reported to OSHA until October 24, 2022.
