29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:  a- 1827 Millcreek Road, Manahawkin NJ: On or about 10/25/22 a worker and the company owner were exposed to fall hazards, in excess of  25 feet to the ground surface below, when working/walking from a two pieces of OSB while they rested, unsecured, on the elevated forks of an all terrain fork truck. This piece of equipment was not specifically designed by the manufacturer to support a scaffold platform.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
