29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition: (a) O-I Glass, Inc. DBA Glass to Glass at 11331 W Dillon Rd in Broomfield, Colorado: On or about October 25, 2022, and at times prior, employees were exposed to fire and explosion hazards when the employer did not ensure that equipment surfaces were kept clean from accumulations of explosive dust.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) O-I Glass, Inc. DBA Glass to Glass at 11331 W Dillon Rd in Broomfield, Colorado: On or about October 25, 2022, and at times prior, employees were exposed to crushing and amputation hazards when in-running nip points on conveyor rollers (Conveyor #15, #16, #24, #165, #3, and #68) were not guarded. O-I Glass, Inc. DBA Glass to Glass acquired Momentum Recycling, LLC in May 2022. Momentum Recycling, LLC was previously cited for a violation of this occupational safety and health standard 1910.212(a)(1), which was contained in OSHA inspection number 1544201, citation number 1, item number 2 and was affirmed as a final order on February 10, 2022, with respect to the workplace located at 11331 Dillon Road in Broomfield, CO 80020.
