29 CFR 1910.22(a)(3):29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice. On or about February 17, 2023 and times prior thereto, employees in the batch house where the walking/working surfaces were covered in large amounts of glass cullet's, and sand were exposed to slip, trip, and fall hazards. Owens-Illinois, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.22(a)(3), which was contained in OSHA inspection number 1585662, citation number 01, item number 001 and was affirmed as a final order on October 12, 2022, with a final abatment date completed on September 30, 2022, with respect to a workplace located at 5200 Beverly Drive, Waco, Texas 76711.

29 CFR 1910.305(b)(2)(i): Metal covers on pull boxes, junction boxes, and fittings were not grounded. On or about February 17, 2023 and times prior thereto, in the Batch house the employer did not ensure the pull box cover plates were placed back on after maintenance was completed working on it exposing employees to electrical shock hazards.
