29 CFR 1926.100(a):Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: On or about October 24, 2022, at a work-site located at 2600 Easton St NE, Canton Ohio 44720; during foundation prep work of an Amphitheater, the employer did not ensure employees working in a trench setting the foundation piece of a pillar were protected from possible danger of head injury from impact, or from falling or flying objects, falling debris from the bucket of an excavator that was lowering the steel foundation piece into place while the employees worked directly under the bucket.
