29 CFR  1926.451(c)(2)(ii): Unstable objects shall not be used to support scaffolds or platform units.  Front wall: The scaffolding platform employees stood on while laying bricks was supported by 2 x 6 lumber that were secured to the scaffolding uprights with wire. Employees were exposed to fall hazards.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:  Front wall: Employees working on the scaffolding to lay bricks used a stairway to access a lower scaffolding platform then scaled the scaffolding braces from the stairway landing platform to access the upper platform. Employees were exposed to fall hazards.

29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.  Front wall: No guardrails were installed on the north and the south end of the scaffolding employees stood on while laying bricks for a column. Employees were exposed to fall hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On the roof: An employee was working on the roof  water blasting, sweeping, and removing debris without fall protection. The employee was exposed to fall hazards, up to 35 feet.
