The flapper was not restrained and/or blocked (i.e. locked out) while task was taking place on the # 5 draw bench machine. An employee suffered a fingertip amputation from the flapper falling downwards.

29 CFR 1910.147(c)(4)(ii):The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy: A) Salem Tube, Inc. on or about October 10, 2022 in the shop area: The energy control procedures specific to #5 Draw Machine did not specify how to control the flapper lid from falling while it is in the raised position (i.e. gravitational energy source). Employees were conducting inspecting/analyzing the flapper on the #5 draw bench machine. The flapper was not restrained and/or blocked (i.e. locked out) while task was taking place on the # 5 draw bench machine. An employee suffered a fingertip amputation from the flapper falling downwards.
