29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  A.) Salem Tube, Inc., on or about 7/17/23 and times prior thereto: employees were operating Draw Bench #9 without guarding to prevent employees from coming into contact with moving parts.  Moving Parts include but are not limited to guide wheels for bars. Pinch point formed by bars moving through the wheels   B.) Salem Tube, Inc., on or about 7/17/23 and times prior thereto:  employees were operating Draw Bench #9 without guarding around the inserter guide that bars travel through to meet the tubes.  Pinch point formed by bars moving through the guide.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  A.) Salem Tube, Inc., on or about 7/17/23 and times prior thereto: employees were performing service and maintenance activities on equipment including, but not limited to Draw Bench #9. When checking alignment, lockout tagout methods and practices were not used to safeguard the worker performing the alignment check.   B.) Salem Tube, Inc., on or about 7/17/23 and times prior thereto: employees were performing service and maintenance activities on equipment including, but not limited to Draw Bench #9. When setting up the machine, lockout tagout methods and practices were not used to safeguard the worker performing the set up.   ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). 29 CFR  1904.29(b)(3):The employer did not enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness had occurred:  A.) Salem Tube, Inc., on or about 7/17/23, an employee sustained a recordable injury while working on Draw Bench #9. The injury was not recorded on the 300 log, within 7 calendar days of the injury.  The Injury was not marked on the 300 log for 46 days.    NO ABATEMENT CERTIFICATION OR DOCUMENATION REQUIRED
