29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) 219 Ct-Rt 57, Phoenix, NY 13135: On or about October 31, 2022, employer failed to ensure each employee working on a walking/working surface with an unprotected side or edge, which was approximately 20 feet above a lower level, was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Campany Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13)), which was contained in OSHA inspection number 1272204, citation number 2, item number 1, and was affirmed as a final order on (2/7/2018), with respect to a workplace located at 110 Second North Street, Syracuse, NY 13202.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) 219 Ct-Rt 57, Phoenix, NY 13135: On or about October 31, 2022, employees using a portable ladder to access a rooftop did not have the portable ladder's side rails 3 feet beyond the access point. Campany Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.1053(a)(1)(iii)), which was contained in OSHA inspection number 1272204, citation number 1, item number 2, and was affirmed as a final order on (2/7/2018), with respect to a workplace located at 110 Second North Street, Syracuse, NY 13202.
