The employer had applied two 18 inch wide aluminum planks (working platforms) on mid-level and one 18 inch wide aluminum plank at a higher-level across 5 ft. wide scaffolding frames. The use of two/one 18 inch platforms left the remaining width of the 5 ft. frame open to the ground below exposing employees to a16ft. fall unto the ground below.
