29 CFR  1926.451(b)(1):Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  a)	Randy J. Smith Construction & Roofing, LLC. On or about November 1, 2022 at the address of 2621 E Harbor Rd. Port Clinton, OH 43452: The employer failed to ensure the supported scaffolding levels from which employees performed framing work was fully planked. The employer had applied two 18 inch wide aluminum planks (working platforms) on mid-level and one 18 inch wide aluminum plank at a higher-level across 5 ft. wide scaffolding frames. The use of two/one 18 inch platforms left the remaining width of the 5 ft. frame open to the ground below exposing employees to a16ft. fall unto the ground below.

Port Clinton, OH 43452: The employer did not ensure that employee(s) operating  aerial lift donned and secured their personal fall arrest systems before operating  JLG aerial lift Model# 600S, Serial# 0300086226.  Employees performing framing work activities (sheeting trusses) on a residential wood frame structure were exposed to falls of 18ft - 25ft or greater to the ground below.

Port Clinton, OH 43452: The employer did not ensure that each employee working from JLG aerial lift Model# 600S, Serial# 0300086226 were trained (written training) in the recognition of hazards associated with those types of mobile scaffold. Employees performing framing work activities (sheeting trusses) on a residential wood frame structure were exposed to falls of 18ft - 25ft or greater to the ground below.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) Randy J. Smith Construction & Roofing, LLC. On or about November 1, 2022 at the address of 2621 E Harbor Rd. Port Clinton, OH 43452: The employer did not ensure employees wearing PFAS atop a wood frame residential structure were connected to an established anchoring point. Two employees performing framing work activities (sheeting trusses) atop the structure were exposed to falls hazards of 17ft - 25ft or greater while wearing PFAS that did not have all the components (a lanyard attached to a vertical/horizontal lifeline) for it to be considered a complete system.   Randy J. Smith Construction & Roofing, LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1567194, citation number 2, item number 1 and was affirmed as a final order on February 10, 2022, with respect to a workplace located at 5671 Commodore Dr., Port Clinton, OH 43452.  Randy J. Smith Construction & Roofing, LLC was previously cited for a violation of an equivalent  Occupational Safety and Health Standard 1926.501(b)(1), which was contained in OSHA inspection number 1462476, citation number 1, item number 2 and was affirmed as a final order on March 24, 2020, with respect to a workplace located at 3495 NW Catawba Rd. Port Clinton OH 43452.
