29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    (a) On or about October 27 2022, at the jobsite located at 403 S. State Street, Oconomowoc WI, employees were performing roofing work on a two story residential house with a pitch of 10:12 with a fall height of 19.3 feet without any fall protection in place and not utilizing fall protection correctly, thereby exposing the employees to a fall hazard.  (b) On or about October 27 2022, at the jobsite located at 403 S. State Street, Oconomowoc WI, employees were performing roofing work on a single story residential garage with a pitch of 10:12 with a fall height of 7.3 feet without any fall protection in place, thereby exposing the employees to a fall hazard.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   (a) On or about October 27 2022, at the jobsite located at 403 West Wisconsin Ave, Oconomowoc WI, employees were performing roofing work on two story residential house and single story garage without any fall protection in place, thereby exposing the employees to a fall hazard. Employees did not have fall protection training.

(b) On or about October 27 2022, at the jobsite located at 403 State Street, Oconomowoc WI, the ladder employees used to access the roof of the residential garage, did not extend three feet above the landing surface.
