29 CFR 1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) On June 1, 2023, employees exposed to struck-by hazards from employees working above them were not wearing hard hats.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) On June 1, 2023, employees performing roofing work were exposed to fall hazards of approximately 23.2 feet.  Giovanni Cazares dba Giovanni Cazares was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1631906, Citation 1, item 1, and was affirmed final order on April 4, 2023, with respect to a workplace located at 403 S. State Street, Oconomowoc, WI , 53066.  No abatement certification or documentation is required for this item.
