Production floor: On or about October 31, 2022, and at times prior, the employer exposed employees to struck-by hazards in that employees were allowed to operate forklifts without the employer first ensuring the employee was properly trained and competent to operate the equipment as demonstrated by the completion of the required training and evaluation specified in the regulation.

Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets. a) Box gluing machine, box assembly production floor: On or about October 31, 2022, and at times prior, the employer exposed employees to caught-in and amputation hazards, in that the side of the box feed way on the 120 gluer machine had an exposed sprocket and chain that was not properly guarded.
