29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Box Assembly Production Floor - RDC Stacker: On or about January 10, 2025, and at times prior thereto, the employer exposed employees to caught-in and amputation hazards in that maintenance employees were allowed to perform maintenance operations on equipment such as but not limited to the RDC Stacker machine without utilizing lockout/tagout energy control procedures to prevent accidental startup of the machine. The employer, Gulf Packaging, Inc., was previously cited for a violation of this occupational safety and health standard (29CFR 1910.147(c)(4)(i)), which was contained in OSHA inspection number 1632278, Citation 1, Item 1 and was affirmed as a final order on January 31, 2023, with respect to a workplace located at 43489 Nicholsville Road, Bay Minette, Alabama 36507.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) Box Assembly Production Floor Bay Machinery, RDC Stacker: On or about January 10, 2025, and at times prior thereto, periodic inspections were not conducted on the energy control procedures for machines such as but not limited to, the Bay Machinery RDC Stacker.
