29 CFR 1926.1051(a):A stairway or ladder shall was not provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: 680 Union St., Springfield, MA: On or about November 3, 2022, an employee was observed climbing the steep embankment at this location leading to the work area and a stairway or ladder was not provided where the break in elevation was more than 19 inches.
