29 CFR  1910.95(d)(3)(i): The employer did not perform repeat employee exposure monitoring when additional employees may be exposed at or above the action level:  a) Production area located at 1800 B Sarasota Business Parkway Conyers, GA; On or about November 7, 2022  and times prior to,  the employer did not repeat employee exposure monitoring to employees exposed to noise above the 8-hour action level of 85 dBA. Noise sampling performed on an employee working near the Baler machine resulted in noise levels  of 257% dose equivalent to a time-weighted average (TWA) of 96.8 dBA, which exceeded the 8-hour action level of 85 dBA. The exposure was derived from a sample taken over 421 minutes.  b) Production area located at 1800 B Sarasota Business Parkway Conyers, GA; On or about November 7, 2022  and times prior to,  the employer did not repeat employee exposure monitoring to employees exposed to noise above the 8-hour action level of 85 dBA. Noise sampling performed on an employee working near the Baler machine resulted in noise levels of 175% dose equivalent to  a time-weighted average (TWA) of 94.1 dBA, which exceeded the 8-hour action level of 85 dBA. The exposure was derived from a sample taken over 425 minutes.  c) Production area located at 1800 B Sarasota Business Parkway Conyers, GA; On or about November 7, 2022  and times prior to,  the employer did not repeat employee exposure monitoring to employees exposed to noise above the 8-hour action level of 85 dBA. Noise sampling performed on an employee working on the dry-end of the Corrugator machine resulted in noise levels of 158% dose equivalent to  a time-weighted average (TWA) of 93.3 dBA, which exceeded the 8-hour action level of 85 dBA. The exposure was derived from a sample taken over 405 minutes.  d) Production area located at 1800 B Sarasota Business Parkway Conyers, GA; On or about November 7, 2022  and times prior to,  the employer did not repeat employee exposure monitoring to employees exposed to noise above the 8-hour action level of 85 dBA. Noise sampling performed on an employee working near the Hogger machine resulted in noise levels of 217% dose equivalent to a time-weighted average (TWA) of 95.6 dBA, which exceeded the 8-hour action level of 85 dBA. The exposure was derived from a sample taken over 404 minutes.  e) Production area located at 1800 B Sarasota Business Parkway Conyers, GA; On or about November 7, 2022  and times prior to,  the employer did not repeat employee exposure monitoring to employees exposed to noise above the 8-hour action level of 85 dBA. Noise sampling performed on an employee working on the wet-end of the Corrugator machine resulted in noise levels  of 389 % dose equivalent to a time-weighted average (TWA) of 99.8 dBA, which exceeded the 8-hour action level of 85 dBA. The exposure was derived from a sample taken over 424 minutes.
