29 CFR  1910.179(j)(2): Frequent inspection. The following items shall be inspected for defects at intervals as defined in paragraph (j)(1)(ii) of this section or as specifically indicated, including observation during operation for any defects which might appear between regular inspections. All deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard:  a) On or about November 8, 2022, at a workplace located at 912 Haase Street, Neenah, WI in the production areas: Frequent inspections of the 30 ton P & H bridge crane were not performed to ensure all the crane's operating components were in safe operating condition.   b) On or about November 8, 2022, at a workplace located at 912 Haase Street, Neenah, WI in the production areas: Frequent inspections of the 70 ton Wiscolift East bridge crane were not performed to ensure all the crane's operating components were in safe operating condition.   c) On or about November 8, 2022, at a workplace located at 912 Haase Street, Neenah, WI in the production areas: Frequent inspections of the 35 ton Demag bridge crane were not performed to ensure all the crane's operating components were in safe operating condition.   d) On or about November 8, 2022, at a workplace located at 912 Haase Street, Neenah, WI in the assembly area: The emergency stop function would not engage on the Jib Crane 6 pendant. Frequent inspections of the 2 ton hoist were not performed to ensure all the crane's operating components were in safe operating condition.

29 CFR  1910.179(j)(3): Periodic inspection. Complete inspections of the crane shall be performed at intervals as generally defined in paragraph (j)(1)(ii)(b) of this section, depending upon its activity, severity of service, and environment, or as specifically indicated below. These inspections shall include the requirements of paragraph (j)(2) of this section and in addition, the following items. Any deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard:   a) On or about November 8, 2022, at a workplace located at 912 Haase Street, Neenah, WI in the production areas: The periodic inspection for the P&H 30 ton overhead crane, in-use in the facility was not completed within the previous 12 months.   b) On or about November 8, 2022, at a workplace located at 912 Haase Street, Neenah, WI in the production areas: The periodic inspection for the Demag 35 ton overhead crane, in-use in the facility was not completed within the previous 12 months.   c) On or about November 8, 2022, at a workplace located at 912 Haase Street, Neenah, WI in the production areas: The periodic inspection for the Wiscolift West 70 ton overhead crane, in-use in the facility was not completed within the previous 12 months.    d)  On or about November 8, 2022, at a workplace located at 912 Haase Street, Neenah, WI in the production areas: The periodic inspection for the Wiscolift East 70 ton overhead crane, in-use in the facility was not completed within the previous 12 months.

Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling:   On or about November 8, 2022, at workplace located 912 Haase Street, Neenah, WI at the Wash Room: The Watertite Wet Location locking blade plug and connector L16-30, powering the mobile wash cart was not installed per the manufacturer's wiring instructions; there was a gap between the screw nut and the rubber housing.

When wet processes are used, drainage must be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats must be provided:   On or about November 8, 2022, at a workplace located at 912 Haase Street, Neenah, WI near the mobile wash cart: The concrete floor on both sides of the mobile wash cart were not maintained in a dry condition due to water dripping from the ceiling.
