29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:  a) Jobsite located at 1164 South 18th Street, Philadelphia, PA.: The employer's designated Project Manager, who had the authority to correct and eliminate hazardous working conditions at the jobsite, did not conduct safety or health inspections thereby exposing employees to hazards of unprotected sides and edges of walking working surfaces at heights approximately 20 feet above a lower level.  Condition was observed on or about November 7, 2022.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Jobsite located at 1201 Vine Street, Philadelphia, PA.:  The Employer did not ensure that employees conducting framing operations on the roof of the residential building, approximately 20 feet above the lower level, were protected from fall hazards by the use of a fall protection system.  Condition observed on or about November 7, 2022.  A Squared Construction LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1530175, citation number 1, item number 1 and was affirmed as a final order on 1/16/2020, with respect to a workplace located at 1524 N. Philip St., Philadelphia, PA 19122.   A Squared Construction LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1491777, citation number 1, item number 1 and was affirmed as a final order on 2/23/2022, with respect to a workplace located at 2100 Lombard St Philadelphia, PA 19146  A Squared Construction LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1434359, citation number 1, item number 1 and was affirmed as a final order on 1/16/2020, with respect to a workplace located at 1524 N. Philip St., Philadelphia, PA 19122.
