29 CFR  1926.451(b)(2):Scaffold platforms and walkways were not at least 18 inches wide.  a) Jobsite: 1901 E York St, Philadelphia, PA 19125- On or about January 10, 2023, the employer permitted employees to work from a supported tubular welded frame scaffold platform, approximately 9 ? inches in width, which was erected within an elevator shaft on the 6th level of a residential construction building, thereby exposing the employees to a fall hazard of approximately 60 feet.

29 CFR  1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  a) Jobsite: 1901 E York St, Philadelphia, PA 19125 - On or about January 10, 2023, the employer did not provide a safe means of access to and from each level of a supported tubular welded frame scaffold, which was erected within the elevator shaft of a 6 story residential construction building, by the use of portable ladders, hook-on ladders or other acceptable means. Employees climbed the scaffold end frames on the 5th level of the building to access the 6th level of the building, which measured 7 feet above the walking working surface of the 5th level, and approximately 60 feet above the ground level through the elevator shaft opening.

29 CFR  1926.1053(b)(1):Where ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a) Jobsite: 1901 E York St, Philadelphia, PA 19125 - On or about January 10, 2023, the employer did not ensure the side rails of the job made ladder on the 5th level of a 6th story residential apartment building extended at least 3 feet above the upper landing surface of the 6th level thereby exposing employees using the ladder to access the 6th level to a slip and fall hazard of approximately 10 feet.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system.  a) Jobsite: 1901 E York St, Philadelphia, PA 19125 - On or about January 10, 2023, the employer did not ensure employees conducting framing activities along open and unprotected sides and edges on the 6th level of a residential apartment building, approximately 60 feet above the ground level, were protected from the falls by the use of a fall protection system.  A Squared Construction LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1599432, citation number 1, item number 1, and was affirmed as a final order on December 27, 2022, with respect to a workplace located at 2100 Lombard St, Philadelphia, PA 19146.  A Squared Construction LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1491777, citation number 1, item number 1, and was affirmed as a final order on February 20, 2021, with respect to a workplace located at 2035 E Lehigh Ave, Philadelphia, PA 19130.  A Squared Construction LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1434359, citation number 1, item number 1, and was affirmed as a final order on January 16, 2020, with respect to a workplace located at 1524 N. Philip St., Philadelphia, PA 19122.
