29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  Employees engaged in printing and bindery activities at 12001 Cary Circle, LaVista, Nebraska 68128, were exposed to amputation hazards in that employees were operating a Paper Drilling Machine(Challenge) Model: MS-10A, Serial #55477, and there was no guarding in place to prevent serious injuries.

29 CFR  1910.219(f)(3):Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed:  Employees engaged in printing and bindery activities at 12001 Cary Circle, LaVista, Nebraska 68128, were exposed to amputation hazards in that employees were operating a Johannisberg. 104 S,  and the sprocket wheels and chains were less than seven feet and were not enclosed.

29 CFR  1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:  Employees engaged in printing and bindery activities at 12001 Cary Circle, LaVista, Nebraska 68128, were exposed to potential fire hazards in that a branch circuit located in the bindery area was not legibly marked to indicate its purpose.

Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.  Employees engaged in printing and bindery activities at 12001 Cary Circle, LaVista, Nebraska 68128, were exposed to potential fire hazards in that a branch circuit and an electrical disconnect did not have sufficient access and working space provided.
