Instances included but were not limited procedures to maintain and/or service the Horizon Binder, model BQ-440.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Employees engaged in media production and assembly activities in the Assembly Department and Warehouse of Turnkey Solutions Corporation located at 12001 Cary Circle, La Vista, Nebraska were exposed to caught-in and laceration hazards in that the employees did not equipment was locked out/tagged out prior to clearing jams. Instances included but were not limited to: i. On or about December 22, 2025, when employees opened interlocked door(s) on the GIMA DVD 879, serial number 87932E0 to clear product jams while the equipment was powered on and ii. On or about December 29, 2025, when employees opened the interlocked cover on the Heidelberg Die Cutter elevator to remove paper from the tray while the equipment was powered on.

29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section: On or about January 8, 2026, employees engaged in equipment maintenance and servicing activities in the Assembly Department of Turnkey Solutions Corporation located at 12001 Cary Circle, La Vista, Nebraska were exposed to caught in, electrocution, and thermal hazards in that the employer did not ensure energy control procedures for the Horizon Binder, model BQ-440 contained clear and specific steps to be followed in order to control hazardous energy. Instances included but were not limited to: i. A specific statement of the intended use of the procedure, ii. Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy, iii. Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them, and iv. Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.

Instances included but were not limited to: i. CEN Electronics industrial battery charger, model 12B0600N2B, serial number 01F00207, where the charger was on the ground approximately 19 inches away from a parked powered industrial truck, ii. Barrett Battery industrial battery charger, model 1B12-765, serial number 30-801V9376-H0, where the charger was on the ground approximately 19 inches away from a parked powered industrial truck, iii. Allied C&D Power System industrial battery charger, model FR18CE640; serial number C PIU851531, where the charger was on the ground approximately 14 1/2 inches away from a parked powered industrial truck, and iv. Hobart Accu-Charger industrial battery charger, model 750C3-18, serial number 490CS43917 where the charger was on the ground approximately 24 1/2 inches away from a parked powered industrial truck.
