29 CFR 1926.502(d)(8): Horizontal lifelines shall be designed, installed, and used, under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintains a safety factor of at least two. a.) Jobsite, 605 Ross Avenue, Wilkinsburg, PA 15221: Observed on or about November 9, 2022, employees working on a low sloped flat roof were not protected from falling by use of a personal fall arrest system. Four employees were working on the roof while their harnesses were attached to a Werner horizontal lifeline which was only designed for two people and the lifeline which was then tied off to a intake ventilation duct and the antenna (anchor point) .

29 CFR 1926.503(b)(2): The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training. The latest training certification shall be maintained. Jobsite, 605 Ross Avenue, Wilkinsburg, PA 15221: On or about November 9, 2022, The employer did not ensure that employees were adequately trained in hazard recognition of working from low sloped roofs.
