(a)	WAREHOUSE BREAK AREA- The break area located above the office exceeded the posted load rating of the mezzanine, exposing employees to injuries from the level collapsing.   (b)	WAREHOUSE- The load rating for the storage shelving above employee work areas had not been determined, exposing employees to injuries from the shelving collapsing.   (c)	BORTAC STORAGE- The load rating of the mezzanine had not been determined, exposing employees to injuries from the level collapsing.

29 CFR  1910.101(b):The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: (a)	WAREHOUSE MUNTIONS STORAGE:  Two cylinders of carbon dioxide were stored without being secured against tipping/falling, exposing employees to injuries from projectiles.  (b)	WAREHOUSE MUNITIONS STORAGE: A cylinder of carbon dioxide was stored with without a valve protection cap on, exposing employees to injuries from projectiles.

(a) BUILDING 3 BORTAC STORAGE- CN grenades, instantaneous grenades, and white star parachute flares were stored improperly amongst flammable materials exposing warehouse workers to potential smoke inhalation and burns.
