29 CFR  1960.8(a): 29 CFR 1960.8(a):  The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm to employees such as crushed by and struck by hazards created by unsafe utilization of racking systems:   (a) 5450 Carlisle Pike, Mechanicsburg PA, Building 305, Shop Stores  - On or about November 14, 2022, the two-tiered rack labeled 20 D 1 B was visibly damaged on one of the vertical supports and was not marked for employees not to use. Employees were exposed to crushed by or struck by hazards.   (b) 5450 Carlisle Pike, Mechanicsburg PA, Building 305, Transportation near Bay E1 and E2  - On or about November 14, 2022, the two-tiered rack labeled 305-K08A01 and 305-K08A02, and also the 4-bay two tiered racking system containing tractor attachments such as the mower blade and the blue plow did not have locking pins installed on racks where items were stored. Employees were exposed to crushed by or struck by hazards.  (c) 5450 Carlisle Pike, Mechanicsburg PA, Building 305, Transportation near Bay E1 and E2  - On or about November 14, 2022, the vertical supports of the two-tiered rack labeled 305-K08A01 and 305-K08A02 was not anchored to the floor. Employees were exposed to crushed by or struck by hazards.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  (a) 5450 Carlisle Pike, Mechanicsburg PA, Building 305, small equipment mechanic shop  - On or about November 14, 2022 the 240 Vac electrical disconnect labeled "Dryer" was blocked from access by a table being placed between the washer/dryer unit and the metal sink. Access was restricted in the event of an emergency.  The US Navy was previously cited for a violation of this occupational safety and health standard 1910.303(g)(1), which was contained in OSHA inspection number 1618388, citation number 2, item number 2 and was affirmed as a final order on 10/13/2022, with respect to the Morale, Welfare & Recreation, Buildings 1261, 1281 and Beach Cottages located at US Navy, Pacific Missile Range Facility,  P.O. Box 128, Kekaha, HI 96752.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
