29 CFR  1926.102(a)(1):Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about November 14, 2022, during the installation of framing material at the job site located at 14860 Private Dr, East Cleveland Ohio 44112; the employer did not ensure employees working on the third floor gable were protected from possible danger of eye injury from impact, or from flying objects from the employees installing framing material with air driven nail guns.

29 CFR  1926.453(b)(2)(iv):Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:  On or about November 14, 2022 at a job site located at 14860 Private Dr, East Cleveland Ohio 44112; during the installation of wood framing material on the third floor of an assisted living facility, the employer did not ensure all employees using an aerial lift stood firmly on the floor of the basket while performing work activities (An employee stood on the midrail of the basket to gain additional reach while installing framing material).

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about November 14, 2022, during the installation of framing material at a job site located at 14860 Private Dr, East Cleveland Ohio 44112; the employer did not ensure employees working on a third floor gable of an assisted living facility were protected from a fall of approximately 31.6 feet by utilizing a fall protection system.   Strano Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(1) Duty to have fall protection), which was contained in OSHA inspection number 1476303, citation number 02, item number 01 and was affirmed as a final order on 6/19/2020, with respect to a workplace located at 27155 Detroit Rd. Westlake, Ohio 44145.
