29 CFR  1926.102(a)(1):  The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  At a residential worksite located at 7025 NW Forest Lakes Dr, Parkville MO -- employees engaged in roofing duties using pneumatic nail guns, hammers and other various hand tools; did not have eye protection on. This exposed the employees to getting objects and/or debris in their eyes. This was documented on or about 11/8/2022.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  At a residential worksite located at 7025 NW Forest Lakes Dr, Parkville MO -- employees were engaged in roofing duties and not utilizing their fall protection system. This exposed the employees to fall hazards greater than 12 feet above ground level. This was documented on or about 11/8/2022.

29 CFR  1926.1053(a)(2):Ladder rungs, cleats, and steps were not parallel, level, and uniformly spaced when the ladder was in position for use:  At a residential worksite located at 7025 NW Forest Lakes Dr, Parkville MO -- employees were using an extension ladder to gain access to the roof. However, the left cleat of the ladder was not flat/parallel with the ground. Thus making the ladder uneven and not parallel to the ground itself. This exposed employees to fall hazards from a ladder. This was documented on or about 11/8/2022.
