This exposed the employees to getting objects and/or debris in their eyes. This was documented on or about 11/8/2022.

29 CFR 1926.1053(a)(2):Ladder rungs, cleats, and steps were not parallel, level, and uniformly spaced when the ladder was in position for use: At a residential worksite located at 7025 NW Forest Lakes Dr, Parkville MO -- employees were using an extension ladder to gain access to the roof. However, the left cleat of the ladder was not flat/parallel with the ground. Thus making the ladder uneven and not parallel to the ground itself. This exposed employees to fall hazards from a ladder. This was documented on or about 11/8/2022.
