29 CFR  1926.502(d)(10)(i): Except as provided in paragraph (d)(10)(ii) of this section, when vertical lifelines were used, each employee was not attached to a separate lifeline:   On or About: November 15, 2022 Location: 412-418 Whiton Street, Jersey City, NJ 07302  a) Two employees were attached to one lifeline performing siding work approximately four stories high. Employees were exposed to fall hazards.

29 CFR  1926.502(d)(11):Lifelines were not protected against being cut or abraded:  On or About: November 15, 2022 Location: 412-418 Whiton Street, Jersey City, NJ 07302  a) Employees were utilizing lifelines that were rubbing against metal framing with sharp edges approximately four stories high.  Lifelines were not protected against being cut, exposing employees to fall hazards.

29 CFR  1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and/or were not capable of supporting at least 5,000 pounds (22.2 kN) per employee a   On or About: November 15, 2022 Location: 412-418 Whiton Street, Jersey City, NJ 07302  a)The employer allowed employees to use anchorages for attachment of personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, nor were they designed, installed, and used under the supervision of a qualified person as part of a complete personal fall arrest system which maintains a safety factor of at least two.  Employees used a rope that was tied around rolled steel metal framing studs that had sharp edges, exposing employees to fall hazards.
