29 CFR 1926.452(j)(2): When the pump jack had to pass bracing already installed an additional brace was not install approximately 4 feet (1.2 m) above the brace to be passed, and was not left in place until the pump jack was moved and the original brace reinstated. On or about: February 28, 2025 Location: 402 Halstead Ave. Harrison, NY 10528 western side of the building a) Two (2) workers utilizing pump jack scaffold approximately 48 feet in height. Worker removed three braces which were anchored into the building to allow the working platform to pass but did not replace braces after moving past the area. The removal of the braces left an approximate 24-30 foot vertical spacing between the braces still in position. Workers exposed to fall hazard.

Workers exposed to fall hazard.
