29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is no more than one inch: (a) Jobsite: On or about 11/13 and 14/2022, employees were working from scaffold levels that were not fully planked. The fall hazard ranged up to approximately 40 feet. The employees were replacing bricks on the side of a building.

29 CFR 1926.451(f)(6):Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6). (a) Jobsite: On or about Sunday 11/13/22 and Monday 11/14/22, employees were working from tubular welded frame scaffolding that was to close to overhead electrical lines. Two employees were working from tubular welded frame scaffold scaffolding and the metal scaffolding was approximately 3 feet away from uninsulated 120-240 vAC lines, about 4-5 feet from uninsulated jumpers (transformers to overhead uninsulated overhead lines) that were 23,000 vAC phase to phase and 13.200, vAC phase to ground and were about 3 feet from insulated 23.000, vAC, phase to phase and 13,200 vAC phase to ground lines. The employees were replacing bricks on the side of a building. The employees were about 40 feet off of the ground on the tubular welded frame scaffolding.
