29 CFR 1960.8(a):  The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm:  Warehouse: On and before November 22, 2022,  employee(s) were loading/unloading industrial rack system(s).  The bases of the industrial rack system(s) were not anchored with anchor bolts to the concrete floor and the industrial rack was missing an under-rack metal brace(s). These conditions potentially exposed employee(s) to rack system(s) tipping over creating crushing hazard(s).

29 CFR  1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  Warehouse: Produce Prep Room:  On and/or before November 22, 2022, employee(s) used a Heat Seal 625A Single 18" Roll Film Axle Mounted Countertop Wrapping Machine to wrap fresh produce for public sale. The extension cord used for power had damage to the jacket/insulation exposing the inner wires which potentially exposed employees to electrical shock and/or fire hazard(s).

29 CFR  1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.  Reference: ANSI Z358.1 Emergency Eyewash and Shower Standard 5.4.4 The eyewash unit shall be located on the same level as the hazard and the path of travel shall be free of obstructions that may inhibit the immediate use of the equipment.  Battery Charging Area: On and/or before November 22, 2022, employees encountered corrosive materials such as sulfuric acid during forklift battery charging operations and spill cleanup procedures.  The plumbed eye wash station next to the charging station was blocked by a Genie GR-20 Vertical Lift and potentially exposed employees to eye burns or blindness.
