Sheffield Village OH 44054, did not ensure employees were protected from head injuries while performing concrete block lifts/hoists in the shop and yard of the facility. Employees were exposed to being struck-by hazards in that employees work directly near large concrete retention blocks weighing 2,200 pounds.

Sheffield Village OH 44054, did not ensure a 20 foot long trucker's chain used for lifting concrete blocks up and out of the concrete pour forms, had legible identification marking stating size, grade, rated capacity, and reach prior to use. Employees working in this condition are exposed to struck-by hazards in that they have no working load capacity information.

Sheffield Village OH 44054, did not ensure all sprocket wheels and chains were enclosed/guarded on the Sterling concrete truck (Plate: OH PKQ5135 VIN: 2FZAAYAN73AK64032 USDOT #1691644) volumetric mixer. Employees were exposed to ingoing nip-points and caught in hazards from the exposed sprocket wheels and chains on the volumetric mixer, while using the concrete truck to pour concrete into forms within the facility.

Sheffield Village OH 44054, did not ensure a DeWalt brand angle grinder (Model: DWE4012) was equipped with the appropriate factory installed guard while in use. Employees using the DeWalt angle grinder in the unguarded condition to cut structural steel and grind welds, were exposed to potential struck-by, laceration and amputation injuries from the unguarded blade during cutting operations. b) On or about November 22, 2022 the employer North Coast Marine Construction Inc. at jobsite 5115 Oster Rd. Sheffield Village OH 44054, did not ensure a Milwaukee brand angle grinder (Model: Heavy Duty Sander) was equipped with the appropriate factory installed guard while in use. Employees using the Milwaukee angle grinder in the unguarded condition to cut structural steel and grind welds, were exposed to potential struck-by, laceration and amputation injuries from the unguarded blade during cutting operations.
