29 CFR 1910.305(b)(3)(i):Boxes shall provide a complete enclosure for the contained conductors or cables. A) On or about November 12, 2022, the employer failed to ensure that boxes containing conductors or cables provided complete enclosure. Within the 8000-pound freight elevator, the Courion Elevator interlock box was missing the cover. Failure to ensure conductors and/or cables are completely enclosed exposes the employees to potential electrical shock and electrocution hazards.

29 CFR 1910.305(i)(2)(i):Fixture wires used for installation in lighting fixtures and in similar equipment were not enclosed or protected and were subject to bending or twisting in use. A) On or about November 12, 2022, the employer failed to ensure that in lighting fixtures where enclosed or protected wiring is installed, it is not subject to bending or twisting. The light fixture located within the 8000-pound freight elevator was not protected from bends or twist. Failure to ensure wiring used in the light fixture is protected from bends or twists exposes the employees to potential electrical shock and electrocution hazards.
