29 CFR 1910.215(a)(4):Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: Employees engaged in grinding activities at the bench grinder located in the Maintenance Shop area were exposed to struck-by and caught-in hazards in that the work rest on the Craftsman 6"-wheel Bench Grinder was approximately 1/2 inch away from the grinding wheel due to severe uneven wear of the wheel which created pinch points when used by the employees.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: Employees engaged in grinding activities at the Craftsman 6"-wheel Bench Grinder located in the maintenance shop area were exposed to struck-by hazards in that the distance from the abrasive wheel to the adjustable tongue guard was found to be approximately 1/2 inch.
