On or about November 28, 2022 at the address of 26760 Lime City Rd. Perrysburg OH 43551: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.

Perrysburg OH 43551: The employer did not ensure each affected employee used appropriate eye or face protection while utilizing pneumatic nail guns/electric circular saw during roofing work activities on residential structure. Employees working in this condition were exposed to potential struck by hazards (flying projectile/ricochet nails).

On or about November 28, 2022 at the address of 26760 Lime City Rd. Perrysburg OH 43551: The employer did not ensure employees were protected from falls while performing roofing work activities (removing old shingles/re-sheathing OSB) atop a single-story residential structure with an 8 -12 roof pitch. Employees operating in this conditions were exposed to falls of 9ft - 16ft - 4" to the ground below.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a ridged support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the the ladder:  a)	Victor Manuel Garcia dba Macri Roofing. On or about November 28, 2022 at the address of 26760 Lime City Rd. Perrysburg OH 43551. The employer did not ensure the aluminum extension used ladder by employees to access the roof level of the structure had its side rails extended at least three feet above the landing surface. Five employees using the ladder in this condition were exposed to fall hazards of approximately 9ft to the ground below.

On or about November 28, 2022 at the address of 26760 Lime City Rd. Perrysburg OH 43551: The employer did not provide a training program for each employee potentially exposed to fall hazards while using an aluminum extension ladder to access a single story residential roof structure. A training program would enable each employee to recognize the hazards of falling from the ladder and the procedures to be followed in order to minimize these hazards.
