29 CFR 1910.147(c)(4)(i) Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. a. Beck Suppliers, Inc. worksite located 80 Hopewell Ave, Tiffin, Ohio 44883: On or about November 30, 2022, the employer failed to ensure energy control procedures were utilized when performing servicing and maintenance on energized equipment such as but not limited to truck mounted power take offs (PTO) and pumps. Employees accessing the pump near the unguarded rotating PTO, were exposed to caught in hazards.

29 CFR 1904.29(b)(1): The employer did not provide the required injury or illness descriptions on the Log of Work-Related Injuries and Illnesses, OSHA Form 300: a. Beck Suppliers, Inc. worksite located 100 North Front St, Fremont, Ohio 43420: On or about December 1, 2022, the employer failed to ensure that the OSHA Form 300 for calendar year 2022 contained a complete description in column F for multiple entries. The 2022 300 log did not define the substance that directly injured or made person ill.
