Beck Suppliers, Inc. worksite located 80 Hopewell Ave, Tiffin, Ohio 44883:  On or about November 30, 2022, the employer failed to ensure energy control procedures were utilized when performing servicing and maintenance on energized equipment such as but not limited to truck mounted power take offs (PTO) and pumps.  Employees accessing the pump near the unguarded rotating PTO, were exposed to caught in hazards.
