29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: a) On or about November 22, 2022. the employer did not protect employees from stepping into unprotected floor holes in the building, floor opening 1 was located at steel column next to 4th deck access. b) On or about November 22, 2022. the employer did not protect employees from stepping into unprotected floor holes in the building, floor opening 2 was located at steel column on east side of deck access. c) On or about November 22, 2022. the employer did not protect employees from stepping into unprotected floor holes in the building, floor opening 3 was located at steel column on east side of deck access next to northeast wall. d) On or about November 22, 2022. the employer did not protect employees from stepping into unprotected floor holes in the building, floor opening 4 was located at steel column on west side of deck access next to south wall. e) On or about November 22, 2022. the employer did not protect employees from stepping into unprotected floor holes in the building, floor opening 5 was located at steel column on east side of deck access next to southwest wall. f) On or about November 22, 2022. the employer did not protect employees from stepping into unprotected floor holes in the building, floor opening 6 was located around rebar located in the middle of deck. g) On or about November 22, 2022. the employer did not protect employees from stepping into unprotected floor holes in the building, floor opening 7 was located around rebar located next to the south wall. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
