29 CFR 1910.147(c)(4)(ii)(B): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. On or about December 5, 2022, at an establishment located at 2200 Larsen Road in Green Bay, Wisconsin; employees performing preventative maintenance tasks were exposed to struck-by and caught-in hazards when the employer's lockout/tagout procedure for the I65 Embosser, Perf, & Winder machine did not identify all sources of potentially hazardous energy such as, but not limited to hydraulic energy.

29 CFR 1910.147(d)(3): Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). At an establishment located at 2200 Larsen Road in Green Bay, Wisconsin; the employer did not ensure that employees isolated machines or equipment from to pneumatic, hydraulic, and electrical energy sources prior to performing servicing or maintenance activities on the machines. Employees used individual tags placed on interlock doors instead of an energy isolating device while performing the work in the following instances: a) On or about December 13, 2022: employees working on the R63 Unwind, Embosser, Perf, Winder & Accumulator machine were exposed to unexpected energization or start up from their machines while conducting servicing and maintenance activities such as but not limited to cleaning and changeover of their machines without controlling all hazardous energies. b). On or about December 5, 2022: employees working on the R64 Unwind, Embosser, Perf, Winder & Accumulator machine and were exposed to unexpected start up from their machines while conducting servicing and maintenance activities such as but not limited to cleaning and changeover of their machines, without properly locking and tagging out the equipment. The machine was being set up to go from one ply to two ply. c). On or about December 8, 2022: employees working on the I65 Embosser, Perf, & Winder machine were exposed to unexpected energization or start up from their machines while conducting servicing and maintenance activities such as but not limited to cleaning and changeover of their machines without controlling all hazardous energies.
