29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of hazardous energy when employees were engaged in activities such as servicing and/ or maintenance of equipment. In the production line, employees engaged in preventive maintenance activities on the Holz-Her Machine - Type 1488, machine #100, edge bender, were exposed to crushing hazards and electrical shock hazards. Lockout /tagout procedures were not being utilized to control all energy sources of the machine prior to maintenance work being performed.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by: In the warehouse, drill press and band saw were not equipped with point of operation guarding, exposing employees to a caught in and struck by hazard in the following instances: 1) The Delta Drill Press not equipped with a point of operation guard around the rotating spindle to protect employees from the hazards of the rotating spindle and the flying chips. 2) The Grizzly Industrial bandsaw had the saw exposed 3.5 inches underneath the cutting table, was equipped with a guard around the rotating saw blades, but was not fitted properly to protect employees from the hazards of the rotating blade. 3)The General, Model # 490 Bandsaw had the saw exposed 3.5 inches underneath the cutting table, was not fitted with a guard to protect employees from the hazards of the rotating blade.
