29 CFR 1910.22(a)(1):All places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. a) On or about December 7, 2023, the employer did not ensure the floor in the powered industrial vehicle (PIV) charging station area was kept clear of cardboard debris, insulation, and other trash to prevent employees who operate PIVs such as, but not limited to, pallet jacks and forklifts, from being exposed to trip, slip, and fall hazards. b) On or about December 7, 2023, the employer did not ensure the walkway to the emergency exit located in the pick area (area with shelving) was kept clear of cardboard debris and boxes to prevent employees who work in the area from being exposed the slip, trip, and fall hazards.

29 CFR 1910.176(b):Secure storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse. a) On or about December 7, 2022, the employer did not ensure that when employees were placing shipping labels on boxes, the stacks of boxes in the ET-6 ARA area were stacked, blocked, interlocked and limited in height to ensure the boxes did not fall on employees due to the load being unstable. b) On or about December 15, 2022, the employer did not ensure that when employees were placing shipping labels on boxes, the stacks of boxes in the ET-6 ARA area were stacked, blocked, interlocked and limited in height to ensure the boxes did not fall on employees due to the load being unstable. c) On or about December 15, 2022, the employer did not ensure that when forklift drivers were driving around the warehouse, the returns stored by loading dock door 3 in the ET-1 ARA BUM area were stacked, blocked, interlocked and limited in height to ensure the pile of return products did not fall on employees due to the load being unstable.
