29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements of subpart I of this part were not worn or attached to the boom or basket when working from a bucket truck. a) On December 6, 2022, at the above addressed jobsite, the employer did not ensure that all persons who operated vehicle-mounted elevating and rotating work platforms, such as Boom Bucket Truck License Plate 14789MJ, 153127H, 14300MJ, 14302MJ, and 14790MJ, donned a harness or body belt and lanyard attached to the boom or basket. No abatement certification or documentation is required for this item.

29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: a) On December 6, 2022, at the above addressed jobsite, the employer failed to require the use of head protection, such as hard hats, for employees that were exposed to falling objects and debris during tree trimming. The employees were observed working below bucket operators that were above cutting branches that fell near the employees' location. No abatement certification or documentation is required for this item.
