Phipps, Inc DBA GH Phipps Construction Companies, 1158 Delaware St, Denver, CO 80204: On or about 12/6/2022, did not ensure employees were protected from crushed-by and struck-by hazards in that a worker was attempting to rig a concrete form that was not properly secured when the hooks from the crane caught a L-bracket on the form and knocked it over onto the worker. This exposed workers to crushed-by and struck-by hazards.
