29 CFR 1910.23(b)(10):The employer did not ensure any ladder with structural or other defects was immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with 1910.145 and removed from service until repaired in accordance with 1910.22(d), or replaced. Located in the maintenance shop, a wooden ladder leading to a loft area was available for employee use. The wood side rail of the ladder had a large split where the metal handrail and the wooden side rail were connected. Employees who work in the maintenance shop were exposed to a fall hazard.

29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. Located in the maintenance shop, a Rikon Model #30-240 20" drill press did not have a guard at the point of operation. Employees are exposed to amputation and laceration hazards.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries. Located in the Park Headquarters building in front of a gray filing cabinet, six portable fire extinguishers were sitting on the floor and were not mounted, located, or identified so that they were readily accessible to employees. Employees are exposed to a fire hazard that could lead to smoke inhalation or burns.
