Facility: Press Area a) Employee were exposed to amputations while operating an CIDAN K25-30 a metal folding machine without guarding; on or about December 13th, 2022. b) Employee were exposed to amputations while operating an FORMA 30 a metal folding machine without guarding; on or about December 13th, 2022. c) Employee were exposed to amputations while operating an CIDAN K15-30 a metal folding machine without guarding; on or about December 13th, 2022. d) Employee were exposed to amputations while operating an EVO 40 a metal shearing machine without guarding; on or about December 13th, 2022. ABATEMENT DOCUMENTATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.

29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. (A) The employer did not establish a program consisting of energy control procedures, employee training and/or periodic inspections as required. Facility: Press Area The employer did not establish a program consisting of energy control procedures, therefore exposing employees to electrical and mechanical hazard while performing maintenance.
