29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On or about December 15, 2022, employees were exposed to caught on and crushing/pinch point hazards while working on and around the Hyundai-Kiamachi CNC Machine (Machine #48 & Model #VX950M). The main access door was not properly guarded to prevent it from being opened during operation. (b) On or about December 15, 2022, employees were exposed to caught on crushing/pinch point hazards while working on and around the Hyundai WIA CNC Machine (Model #F650/50 & Serial #03692-0446). The main access door was not properly guarded to prevent it from being opened during operation. (c) On or about December 15, 2022, employees were exposed to caught on and crushing/pinch point hazards while working on and around the Hyundai WIA CNC Machine (Model #500 & Serial #G3684-3084). The main access door was not properly guarded to prevent it from being opened during operation. (d) On or about December 15, 2022, employees were exposed to caught on and crushing/pinch point hazards while working on and around the Hyundai WIA CNC Machine (Model #L700 LA & Serial #G3229-0021). The main access door was not properly guarded to prevent it from being opened during operation. (e) On or about December 15, 2022, employees were exposed to caught on and crushing/pinch point hazards while working on and around the VMC CNC Machine (Model #VMC4024 & Serial #9-90-98-1462). The main access door was not properly guarded, in-that the interlock installed was not functioning. (d) On or about December 15, 2022, employees were exposed to caught on and crushing/pinch point hazards while working on and around the Green Hydraulic Press. The point of operation created between the bottom die and top die was not guarded.
