29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. (a) On or about December 12, 2022, the machine specific procedure for Cold Form Machine 1 did not include gravitational energy sources and steps to isolate gravitational energy sources, exposing employees to caught-in, struck-by and amputation hazard. (a) On or about December 12, 2022, the machine specific procedure for Cold Form Machine 2 did not include gravitational energy sources and steps to isolate gravitational energy sources, exposing employees to caught-in, struck-by and amputation hazard. (a) On or about December 12, 2022, the machine specific procedure for Cold Form Machine 3 did not include gravitational energy sources and steps to isolate gravitational energy sources, exposing employees to caught-in, struck-by and amputation hazard. 29 CFR 1910.147(c)(7)(iii)(B) Additional retraining shall also be conducted whenever a periodic inspection under paragraph (c)(6) of this section reveals, or whenever the employer has reason to believe that there are deviations from or inadequacies in the employee's knowledge or use of the energy control procedures. (a) On or about December 12, 2022, an employee was unjamming the tail grabber assembly of the Cold Form Machine 1. The Cold Form Machine 1 was not deenergized, locked out, and the tail grabber assembly was not blocked to prevent the tail grabber from sliding down back into the start position. This exposed the employee to a struck-by and crushing hazard. The employer failed to implement energy control application steps as the machine was not isolated from all energy sources [per the 1910.147(d)(3) requirements]. As a result, the remaining applicable energy control elements, involving LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine hazards.
