What this inspection record means

OSHA opens inspections for many reasons — routine scheduling under a national or local emphasis program, an employee complaint or referral, or a follow-up after a reported injury. Opening or conducting an inspection is not itself an allegation or a finding that this employer broke any rule; any findings appear as the citations listed below, and citations can be contested, reduced, or withdrawn.

29 CFR 1910.147(c)(7)(iii)(B) Additional retraining shall also be conducted whenever a periodic inspection under paragraph (c)(6) of this section reveals, or whenever the employer has reason to believe that there are deviations from or inadequacies in the employee's knowledge or use of the energy control procedures. (a) On or about December 12, 2022, an employee was unjamming the tail grabber assembly of the Cold Form Machine 1. The Cold Form Machine 1 was not deenergized, locked out, and the tail grabber assembly was not blocked to prevent the tail grabber from sliding down back into the start position. This exposed the employee to a struck-by and crushing hazard. The employer failed to implement energy control application steps as the machine was not isolated from all energy sources [per the 1910.147(d)(3) requirements]. As a result, the remaining applicable energy control elements, involving LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine hazards.
