What this inspection record means

OSHA opens inspections for many reasons — routine scheduling under a national or local emphasis program, an employee complaint or referral, or a follow-up after a reported injury. Opening or conducting an inspection is not itself an allegation or a finding that this employer broke any rule; any findings appear as the citations listed below, and citations can be contested, reduced, or withdrawn.

29 CFR 1910.145(c)(3): Safety instruction sign(s) were not used where there was a need for general instructions and suggestions relative to safety measure(s): The employer is failing to protect employees from exposure to struck-by, crushing, pinning, pinching, and smashing hazards due to bypassing the electrical interlocked on the door. This was most recently documented on or about November 22, 2023, and time prior thereto, when the employer did not ensure that the heads of the nozzles would not come down to smash employees' hands as they were making adjustment/alignment so that the heads of the nozzles would line up with the holes on top of the batteries while the safety door/guard was open for set-up. The employer did not ensure to disengage the movement of the heads as the safety door was open, resulting in pinching the employee's finger, and amputated the tip of his middle finger.
