(a) J.R. Filanc Construction Company, Inc. on 5445 County Rd 2 in Northglenn, CO: On or about December 15, 2022, and at times prior, employees were exposed to cave-in hazards while grading bedding sand in an unprotected trench that was 4.9 feet deep. The competent person did not properly inspect the trench prior to employees entering the trench. The trench had near vertical walls, visible cracks and fissures, a width of only 2.5 feet, and there were several areas where rocks and clumps of dirt protruded from the trench walls.
