29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Basement Mechanical Room: The employer did not developed each energy source by establishing the step-by-step energy control procedure for the task of repairing a valve on the steam station to prevent the releases of all hazardous energy source(s) to the employees, such as but not limited to the energy source of super-heated water. On November 11, 2022, an employee was transported to Bridgeport Hospitals Burn Unit after sustaining serious burns during the removal of valve from an active steam system.
