Location: Plant floor, 100 Hale Street, Newburyport a) On or about 12/7/22, the employer did not ensure that employees who remained behind to fight a fire did not evacuate after the fire had grown beyond the incipient stage. Employees remained in the facility to extinguish a fire in an oven.

29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting: Location: Plant floor, 100 Hale Street, Newburyport a) On or about 12/7/22, the employer did not ensure that employees who remained behind to fight a fire were adequately trained in the use of portable fire extinguishers. Employees were trained through a video that only contained general information on the use and limitations of fire extinguishers. Employees fought a fire beyond the incipient stage.

29 CFR 1910.147(d)(4)(ii):Lockout devices utilized were not affixed in a manner that held the energy isolating device in a safe or off position: Location: Plant floor, 100 Hale Street, Newburyport a) On or about 12/8/22, the key remained in the lock of a lockout device that was applied to an industrial oven that had been involved in a fire, and the employee who applied the lock was not at the facility. By not securing the machine in a locked out position or physically disconnected from power, the employer did not ensure that the machine remained in a de-energized state.
